Before participating in any physical activity, assessment, or testing, each member must complete the PARQ Form.

All forms MUST be completed and submitted before participating in any EatWater Training, Assessments, or Testing.

***If you schedule a Fitness & Mobility Assessment you must complete all forms prior to the start of the assessment*** (Failure to do so will result in the Assessment being rescheduled).
